You should complete this checklist if you meet the following criteria:

• You are within 8 credit hours of completing Associate of Science in Business or Associate of Science in Education at UAHT.
• You have at least a 2.0 cumulative grade point average in all Business Division courses.
• You must meet specific grade point average and course grade requirements for the BA in K-6 Elementary Education.
  
  

• STEP 1: Attend UAM Informational Session.

  • UAM will be on UAHT campus to assist with registration.
    
    

• STEP 2: Apply for admission to UAM.

  • Complete and submit the UAM Undergraduate Application.
  • Submit required documents to the UAM Office of Admissions:
    • Official UAHTtranscript and transcripts from any other colleges/universities you’ve attended.
    • High School transcript with GPA;
    • Placement test scores;
    • Proof of immunization against measles, mumps, and rubella.
  • In addition, international students must provide:
    • Current tuberculin skin test;
    • Official English language competency test results;
    • Financial affidavit.
  • Other items could be needed as UAM works with UAHT to comply with federal guidelines.
    
    

• STEP 3: If you receive financial aid, apply for financial aid at UAM.

  • Complete Section I of the UAM Consortium Agreement.
  • UAHT should be listed as the Host School on the Agreement form. Your financial aid will come through UAM, which is your Home School. The UAM Financial Aid Office will disburse your financial aid each semester
  • You must file a FAFSA and complete the required financial aid process prior to all applicable deadlines. The FAFSA code for UAM is 001085.
  • You must be enrolled in a degree program at UAM in order to receive financial aid.
  • You must be enrolled in at least one upper-level course at UAM each semester in order to receive financial aid for courses taken concurrently at UAHT. 
    
    

• STEP 4: Be advised by UAM.

  • Contact the advisor shown below for the UAM degree to which you are transferring.  During your advising session, your UAM advisor will complete Section II of the UAM Consortium Agreement, which lists the course(s) you will be taking at UAHT.  Your UAM advisor will forward your agreement form to the UAM Financial Aid Office.  The UAM Financial Aid Office will contact UAHT Enrollment Services for confirmation of enrollment at UAHT.

• STEP 5: Register for UAM course(s).

  • Approved by your UAM advisor in Step 4.
  • You must be enrolled in at least one upper-level course at UAM each semester in order to receive financial aid for courses taken concurrently at UAHT.
  • UAM will be on UAHT campus to assist with registration.
    
    

• STEP 6: Be advised by UAHT.
  

  • Meet with the appropriate UAHT Faculty Mentor listed below to review your UAHT/UAM degree plan and to discuss the courses approved by your UAM advisor in Step 4.

• STEP 7: Register for UAHT course(s).
  

  • You may speak to the UAHT dean to register for classes..
  • Remember that you must also be enrolled in at least one upper-level course at UAM each semester.
