On your initial visit to UAM Student Health Services, you will be required to fill out a Health History Form.

Drop-ins are welcome.  However, to reduce your waiting time, you may call or email for an appointment.

Available Hours:  Monday - Friday; 8:00 a.m. - 4:30 p.m.
