ASCA forms are available as Word Documents that might be opened and/or saved to your computer to be completed and mailed. The are also available as .PDF Documents that may be filled in (see instructions below), printed and mailed.
All ASCA forms are interactive Adobe PDF forms that may be completed within your Adobe Reader before printing the form.
Any form may also be printed without completing the form.
Click the desired .pdf form.
To complete the form before printing, select the "Hand tool" by clicking on the open hand icon on the toolbar.
The pointer changes to the open hand icon.
To check a box, move the icon over the box and click when the icon change to a closed hand with pointing finger.
To remove a check, move the pointer over the box and click.
To enter text, move the icon to the appropriate text area, the icon will change to an I-beam.
Click the mouse and type in the desired information.
When finished with that text area, move the I-beam off the text area until it changes to the hand icon and click.
Select next text area and repeat until form is completed
Print completed form.
