This agreement is entered into this ___ day of ________, 2008 between ______________________________________ (named hereafter in this document as the “Organization”) and THE BOLL WEEVIL, the University of Arkansas-Monticello yearbook, for the purpose of being included in the 2008 BOLL WEEVIL.

 The Organization agrees to furnish:

1. A group photo. (A student publications' photographer can take a group photo by appointment. Call 460-1997 for more information.)
2. Five-to-10 candid photos including names of members in picture if there are less than six.
3. A list of officers with their phone numbers and e-mail addresses.
4. A complete list of members of the Organization typed in alphabetical order with e-mail addresses.
5. A narrative description of the purpose, goals and current activities of the Organization, including those projects or activities UNIQUE TO THE CURRENT YEAR.
6. Any available brochures or written material descriptive of the organization.
7. A quote from an officer or member answering the following question as related to the Organization: How is your group Breaking Ground this year?

Note: The BOLL WEEVIL reserves the right to edit any copy submitted and remake photographs as necessary. The BOLL WEEVIL cannot be responsible for errors when copy is illegible. All photos and copy submitted become property of BOLL WEEVIL except where otherwise noted.
