Required Forms for All Projects

c  Student Entry form – typed

c  Checklist for Adult Sponsor (1)

c  Student Checklist (1A)

c  Research Plan (attached to Student Checklist (1A))

          The research plan for all projects is to include the following:

A.   Question being addressed

B.    Hypothesis/Problem/Engineering Goals

C.   Description in detail of method or procedures

D.   Bibliography (List at least five (5) major references.

c  Approval Form (1B) with:

1.     Signature of SRC committee chair when projects involve (humans, vertebrates or potentially hazardous biological agents)

2.     Date of approval BEFORE experimentation begins.
